Overlay programs are available to all patients currently enrolled in BloomCare's core care management programs — CCM, RPM, TCM, BHI, or any Bloom Series™ specialty pathway. Each program is configured during onboarding based on the practice's patient population and clinical priorities. BloomMeds™ and BloomNutrition™ are available across all patient populations. BloomMove™ is optimized for older adult and mobility-limited chronic disease patients. BloomFunction™ targets patients with functional limitations requiring structured recovery support. BloomWounds™ is available for practices managing patients with chronic wounds as part of their ongoing care needs.
